Hours templates

I intend to keep a written record of my hours worked ideally on a printed spreadsheet, for my own quick reference - are there any good ones out there which people on here use?

Looking for it to include the obvious bits e.g. break times / other work completed etc

Be easy enough to " make " a template to suit yourself.
