First Medical Renewal?

Hi,

I passed my class 2 in february 2017 when i was 49 years old, I’ve been driving since that date, do I need to get another medical because i’m 50 on October 14th?
If so what forms do i need ? D2/D4 or just a D47PU? The date on my card says 2017 - 2022
Do i need a licence renewal form as well as a medical form?

The date next to cat C on your licence is your next medical renewal date

You are not the only driver who thought that

D4 medicals for you if no medical issues will be age 49 54 59 64 then just before age 66
