Mainly because you used the phrase ‘muffed the job up’ in your opening post which made me giggle.
One small critique, Overtime- at my place a reasonable amount of overtime is expected but what reasonable means is anybodies guess. Apart from that a well put together survey with no visible muffs.

Hmmm, good question. I’d say go for it because you’re still an “employed” driver in that you’re driving vehicles which don’t belong to you. Just make it clear on the form if/where appropriate.

Might seem a daft question, but I’m happy to submit my answers as an employer. i can pretend to be one of the drivers. Not sure what i should put in the are you happy bit, though given that they tell me they are and they won’t leave I can take a guess.
And there isn’t, unless I missed it, a space for things like our daily meal allowance and any other quirks to the pay structure.
