The OP is a little sketchy, it’s like a dot to dot with half the dots missing. A D4 medical is only required periodically or to declare a significant change in your health.
The section that says “Are you currently taking Medication” I don’t think Brufen for toothache would be classed.
That is Section 8 - Medication. I was specifically referring to Section 4 - Psychiatric Illness.
