Had this happen to me, but without the tragic consequences. I’ve seen lorries with little screens attached to the door pillars before, but they still had the mirrors on as well. Are mirrors compulsory, even when they’ve been replaced with technology?
I experienced this driving Volvo FH12s in the early noughties for Reed Boardall. Not taken much notice of them now but back then the main and wide angle mirror was one unit covered as one about a good foot long and quite wide. Coming up the sliproad off A1M northbound at Boroughbridge, checks roundabout is clear as I’m approaching end of slip road, sees nowt, pulls out, find a car right in front of me. And it happened more than once. Car was coincidentally going round the roundabout at a speed that meant it had been obscured by the drivers side mirrors as I’d approached.

If it happened more than once then maybe you weren’t looking properly …
from that newspaper article: Mr Gordon (prosecuting) said: “These particular wing mirrors, these double wing mirrors on HGVs, they’ve been a standard feature of HGVs for ten years now, haven’t they?” Mr Gordon asked. “That is my understanding, yes,” said Mr Cass.(CCTV Analyst) “These wing mirrors are a potentially fatal feature of all HGVs driving up and down our roads every day, aren’t they?” Mr Gordon asked. “That’s not for me to comment on,” Mr Cass said.
So our mirrors, fitted for safety reasons, are now potentially fatal? Cass did well to deflect that crass leading question.

This is all back to the old chestnut - things are safe when it suits and dangerous when it suits more [emoji849]

I’m coming to the conclusion that you don’t actually drive a lorry at all and never have.
