Which form is it you need to get your provisional cat C? The D9 or the D2?
It’s just theres boxes to tick on the D9 to add higher catergeries but I’ve also read you need a D2■■?
D2 is the form you need. It usually comes in an envelope with a D4 medical form as well as a booklet explaing how to fill the form in.

IF your last medical for your D was LESS than 12 months ago, then NO - if over 12 months ago - YES
