What failed here then ?
Common sense
Poor bloke but just how well was the load secured !!
never drive oiled Steel

Newbie here, no idea what that is.
Steel that is wiped over with oil to prevent it rusting.
Looks like a failure in the trailer headboard assuming that the piping was loaded butted up directly to the headboard.
Back in the 60s I used to stay in digs in Leeds. The road ran downhill to a T junction with a busy main road and a zebra crossing. One morning I followed an artic loaded with 10 steel pipes in 4,3,2,1 formation. When he got to the junction a woman with a pram made to step out and the driver made a sudden stop with a similar result to the video above. The headboard stopped all but the top three pipes and they just sliced through the cab and ploughed into the main road.
Luckily, the driver sensed/felt/heard it coming and threw himself across the seats. He was completely unhurt but it took the fire brigade a while to get him out. No one else injured either.
These types of high risk (with regard to shifting) loads should by now only be moved with specially constructed, reinforced trailers - but then, i`ve always been a pipe dream, ideal world kind of soul.
Cant see why the fella anchored on tbh, but that
s purely academic now. RIP old mate.

He means “pickled” steel. Pickled being what he mostly is I would imagine.
Can’t see any kindof restraint although suspect it wouldn’t have helped much, but this is China where H&S is pretty much zero. Specialist trailers likely don’t even exist over there and I wouldn’t be surprised if the trailer is rusted to heck to start with. Very unfortunate.
Is there a driver in there or was it for show just looking at the vid looks like the vans watching the truck maybe remote control just there was no reason for him to slam on like that

Yeah - confusing piece of footage really. Looks like the road could be closed, but there`s an oncoming cyclist to the (our) nearside as well■■?
could they be traffic lights on the gantry ?

Yep - quite possibly. Didn`t clock that one. I just noticed that a second or two before the load punches through, the vehicle filming it shuffles forward a couple of feet, then stops again. Curious.

Probably no headboard on the trailer.

Nearly, the term in the trade is P&O or pickled and oiled. The hot rolled steel is put through an acid bath to remove the excess carbon from the surface, then put in an oil bath to coat it and stop it rusting.
It is slippery stuff, but not as bad as thick galvanised oiled steel sheets.

I think that’s the headboard you can see on top of the flattened cab in the stills.
Saw a similar thing when I was a youngster at Levens junction on the A6, when the M6 stopped at Carnforth and all freight went that way. Wagon carrying steel coming down to the traffic lights pulled up a bit too sharp. Nasty.

The other oddity is the entire lack of any other traffic.
It is truly a horrible incident and goes to show how important it is to make sure heavy loads are secured down.

Story and different angle here
The piece under the video says “the driver put on the emergency brake”, hardly surprised that load shifted, if he banged the handbrake on at that speed!
