If you scroll a bit when you land, you’ll see that Sections 26-28 of the Goods Vehicle (Licensing of Operators) Act 1995 deals with suspension, revocation, curtailment and disciplinary stuff, and maybe the link might make a good bookmark for you??

Had a look at A&D’s and thats a full set of offences, including from what I see the new TM repute.

I have no idea about current cases but in my experience this is the sort of thing you see where maintenance has slipped - OCRS below national average or some bad roadside checks. Looking at the region in questions last decisions I would say they are on a crack down as they appear to be revoking a lot of licences at present.

Just worked on a fleet inspection for someone and the VOSA guy goes straight to the PMI’s then asks for Drivers Defect Report (DDR) for the last usage of that vehicle prior to PMI. If the PMI has say a bulb out showing or loose mudguard and the DDR says all OK they will pull you up on DDR’s not being carried out properly.
