Just send a new form and explain it was an oversight on your part. They normally respond. It does happen

Did they renew a photocard licence ? - if yes then that is a bit odd
I hope they scanned it before sending…
Why wouldn’t any one renew all categories if there was nothing more than a tick required to do it, seems a strange question.
I don’t think it was intentional
On the bit of the form where you Tick the boxes to say if you want to renew with or without are the top 2 boxes to renew with and the bottom 2 boxes to renew without or the other way round? He tells me he can’t remember whether he did it the right way round
If they have been removed he will have to get another form off the DVLA to reapply for the entitlements.
I removed the hgv entitlement off my licence last year, after an online licence check showed that my licence had expired, i needed a medical to renew it, I filled in the D4 to remove it and received new licence with hgv removed.
About 6 weeks later I took a medical and sent that off with the form and they sent new licence back with hgv back on, I did it that way as I also needed a medical for my taxi licence so delayed the truck medical until the taxi one was needed,
So it’s just a case of fill out the form and sending it again then. Nothing more complicated than that

It should be, but it is the DVLA, I think expired/removed vocational entitlements will take a while to drop off the system.
A phone call or email to them would confirm the situation for you, I contacted them myself before removing the expired entitlement.
You would need to speak to them to get a new D4 sent out, they will tell you what you need to know.
