Quick Provisional Question please

Hi Guys, Maybe I have just confused myself completely but I’ve filled in my provisional and its ready to go. Do I need to have my D4 medical form before sending off? The D2 says I may need a D4 but doesn’t say if its to be included?

Also, is the provisional entitlement free? I’ve read the form and surmised that it is. But I find it hard to believe the money grabbing ■■■■■■■■ wouldn’t charge for additional prov entitlements??

Send both D2 and D4 together - tick C & D only on D2 - pay no more than £60 for medical
no charge for D2

I just used my village GP and instead of giving him money he said just put £50 in the charity box in reception as it only took 15mins.

My GP wanted the full £150, so it may be worth checking both.
