Plagued by punctures

Scenario - Cut to sidewall of tyre, driver ignores it, the tyre blows and causes a pile up on a motorway. Very easily could be turned into a fatal accident.

Not sure this was the cause,but the type of accident that happened south of J15 on the M6 a couple of months ago.
Trailer tyre blowout,car swerves to avoid tyre debris,takes out three bikers,killing one of them.

Notwithstanding the drivers walkaround checks,which should also be at the end of a shift as well as the beginning.
It’s no good going to pull a trailer off a bay to find you have a tyre problem that should have been defected by the last driver.
Our depot to depot deliveries are time sensative so we can’t afford down time for any reason,and as far as tyres are concerned,we have contract tyre fitters on the yard all the time checking any tractor or trailer that’s not moving,so any tyre trouble en route is very rare,which raises the question,why are there so many tyre problems on the road.
Most tyres are of a quality these days,so if there are so many problems on the road,can it be down to quality of checks?
