Hi in 2014 work sent me for my class c lgv in September 2014 I passed the theory modules 1 and 2. I failed my practical driving test but in December 2014 I passed my module 4 practical cpc. Now work want me to try again but wonder if anyone knows if I need to do modules 2 and 4 again or just the multiple choice and hazard perception.

All theory tests are valid for 2 years - so it’s not looking good. Sorry.

just called driver cpc helpline and got a lady who had been asked this very same question a few days ago when she double checked it with standards 
Answer is
mods 1a + 1b need passing again before doing mod 3 test
mod 2 has been converted into a mod 4 pass so remains valid and a DQC will be issued when mod 3 passed which will be valid for 5 years from the date of the mod 3 pass
That’s great, I did think it would work like that because I had a proper pass certificate for the mod 4. But was given conflicting advice from some supposedly in the know
