Are you sure you need one?
gov.uk/renew-lorry-bus-coac … 45-or-over
“If you’re 45 or over
Because of coronavirus (COVID-19), you currently do not need to provide a medical report (form D4) to apply to renew your licence.
If your application is successful, you’ll get a 1 year licence.”

Note also
gov.uk/government/publicati … ng-licence
“As of 20 June 2019, DVLA no longer accepts medical applications (D4) completed by Doctors on Wheels Ltd.”
If they are still around?

So, the website is still up and running.
However the link I gave says, and I will repeat the quote,
“As of 20 June 2019, DVLA no longer accepts medical applications (D4) completed by Doctors on Wheels Ltd.”
Anyone wishing to use them can do so, but since that quote is from the DVLA site today, why would they?

You’re better going to your own g.p cheers Ray
