Do you mean your first medical to apply for the LGV licence or do you mean a medical renewal 
To obtain a HGV licence (provisional or full) you will need to pass a HGV drivers medical and complete form D4 if you haven’t already.
Sorry I need a medical as I’m being taken on as an apprentice, the company said they will sort out all the paperwork so do they mean the d4? Thankyou

Assuming you will be driving HGVs it will be form D4
You will also need to fill in a D2 form to apply for the relevant provisional eneitltments.
The D2 & D4 (medical) forms will need to be submitted to DVLA, along with your current driving licence.
