Just received my D4 medical form to renew my license. It came with a D47PW form as well. Do I send both forms to the same address? The notes that came as well are not very clear.

Yep, send everything to the same address in the same envelope along with payment if updating photo…of course it has to be a cheque as we still live in 1970.
Just make sure you read the D47PW form booklet for every question as there seem to be questions on there designed to catch you out (proof that DVLA are a bunch of sadistic gits 
). Eg, the one about renewing licence and the one about updating photo - it seems you only tick box A not B even if you are updating the photo as B means you are not renewing (according to the booklet).
Forgot, also refer to the notes onnthe back of D47PW as obviously putting them all into rhe booklet is far too logical. Yes the notes are ■■■■!
