A D48 licence renewal form.
When I got my renewal forms from the DVLA they also sent me a questionnaire that took some time to understand, and asking you if you object to your doctor being informed of any decision they may make on your medical assessment.

If still a couple of months before renewal due and not had a licence issued after october 2012 then change the photo and get 5 years LGV for free
