I passed my HGV 1 & 2 in 2010, I never took a job driving but thought it would be nice to have it on my licence. I moved address a few months ago, my new licence came back without these entitlements on it. Is this due to not having a medical?

Then you need a medical.
A successful medical should reinstate the entitlement, in around 3 years when DVLA get around to it
