I believe when you pass C, you automatically get C+E provisional.
Tick D as well, it won’t cost you anything!

When you pass your C as mentioned CE provisional is automatic. Don’t forget to send your current photo card off
You need to tick provisional C. Ticking D is a good plan. Ticking anything else is a waste of your biro.
Don’t forget to send your D4, medical report, with it together with your current licence.

By ticking D as well as C means the medical will always cover both LGV + PCV so if in the future you decide to do PCV a new medical will not be needed
I applied online for my D2 and D4 forms 13 days ago and received nothing as yet. It said ‘thankyou for submitting, blah, blah, and the forms will arrive in 10 days time’. 
I’d apply again or go down to your local post office. I applied around the same time as you and mine arrived within 4-5 days.
