Left my tacho card in overnight by mistake 
Finished work at 12 midday and its been in on other work until i started the next morning at 6am.
What is the procedure for sorting this out?
Generate a print out of the relevant day, write explanation on the back and sign. Do this twice, you keep a copy and your office gets one.
Print out and write you sins on the back 
keep it with your other records if pulled by dsa (vosa) you can show them you know you made the mistake 
bend over bad boy, all ours default to sleep when switch off

You have just put yourself and others drivers at your place out of work, instant O license removal.
I did it once, did a printout and wrote something like “mode switch error, rest commenced 14:30” on the back, I was stopped by Police/VOSA a few weeks later, copper picked this up on his computer, showed him the printout, he wasn’t remotely bothered. It’s pretty obvious from the analysis that this is what had happened.
