As far as I’m aware you will usually get that error if the card is inserted the wrong way round in which case I’m sure you would have corrected the problem, so the most likely alternative is that the chip needs cleaning or or it’s faulty, you should try cleaning it with a soft cloth if you haven’t already.
If you can, try the card in another vehicle to check that it’s the card rather than the tachograph, alternatively try downloading it to see if the card reader can read it.
After cleaning it if it can’t be read by another tachograph or the card reader then I’m afraid you have a faulty card.

If the card is faulty you should report it to DVLA within 7 days (or better still immediately), you can drive for 15 days but should take vehicle printouts at the start of the shift and another at the end of the shift.
These printouts should be signed and you should write your driver card number or driver license number or your name on them both.

also is it in date ?, check out the expiry date on the card
tried it in another vehicle and also tried cleaning it .been on to dvla and they told me to get a d777b form n send it back to them they also gave me a reference number so if i get a tug i can prove its been reported to them
