As in the title. Does anyone know the address to send completed D4 form and licence application form to? I’ve just moved house and cannot find the envelope that came with the form.

Not unless you’ve had the reminder from them - there’s a date on the front where it says 4a & 4b - that tells you the valid from and expiry date of your current card.
They will use your current photo and digital signature on your provisional when they send it back
no, but mine came back valid for only a year as my photo was old. so may be worth doing it.
