A quick look at this says you don’t need one if you have a CMR. I will be back soon.

I’m guessing here, but I think the full name is as above and it looks like the letter of attestation which you will need.

Sorry. Wrong altogether. I have done France for 4 years on two separate occasions and never heard of one, but David Lowe has 
It seems all that information with the exception of the VAT Number can be written onto a properly completed CMR which could be the reason I have never seen one.
We used a new format of CMR and all the information asked for was printed on the CMR which was also used for internal work and Cabotage. I have blanked out the company information so it doesn’t fall into enemy hands.
Halfway down the page, you will see the Document De Suivi, that is why I never noticed it, just filled it in automatically.
I suppose the German thing for the simplified documents would be a Nahvekehr or Fernvekehr Frachtbrief

I carry a document to prove say that I’m employed
and authorisation to drive the vehicle from the owner.
Letters of attestation, when required.

I don’t know is the honest answer. We were given new CMR books and never really questioned it, but I am guessing it is something to do with Cabotage, especially as in France you have to carry all the counterfoils from the past month or so.
As David Lowe had written it, the only thing missing from a properly completed CMR is the VAT number. Has Jimboys document got that on?

Yes the copy I scanned in has the full address and TVA number hidden under the Spar debit card slip

no never even seen one or had any of the drivers asked for one
Never heard of it, never seen one, never been issued with one, never been asked to produce one…
