We got loads at our office in Coventry.
VOSA LGV test stations have them - well, the ones I use do.
Not sure if post offices have them - but worth asking.
I THINK that you can get them by post from DVLA - not sure on that.
Bet others on here can tell you the easiest way 
From the transport office website:
Digital Tachograph driver and company card application packs are now available from the Driver & Vehicle Licensing Agency (DVLA) - please call 0870 8501074. Workshop card application forms are now available from VOSA - please call 01792 454336.

call in at any agency as they all have them or at least every one i have been to as they want you to have one, they wont pay though lol
