Hey all. Just had my medical done, but couldn’t remember my family doctor’s name at the time :laughing: so I’ve got the form with me at home. Just wondering, does the “E-mail” line need to be filled in? I’ve checked on the practice’s website and found jack. Don’t particularly want to call them up to find out. (Planning to tell them it’s for a job if I have to call :unamused:)

Have you tries googling the practice name and email contact?

If there does not seem to be one then put not available on the D4 form

I just left it blank on mine. Couldn’t find any email addresses on my practice web site and it’s a right pain to get hold on anyone on the phone to ask…So didn’t bother. Came back fine :smiley:

Thank you very much gents. Sent off with “Not available” in the box. :sunglasses:

Gonna have to resort to my passport from when I was 8 if I get asked for ID up the pub :smiley:
