Cat C and CPC Passed & Quick Question

Hi everyone,
Well last week I passed my Cat C test and yesterday I completed my CPC by passing the module 4 test. I’m ready to send my license off now to get the relevant changes made - do I need to complete a form to do this or do I just send the pass certificate and the license as it is now?

I’m also wondering about getting a tachograph card, do I have to wait for my license to come back before applying for one or can it be done at the same time?

One final thing - will I automatically be sent a DQC now or do I need to notify somewhere that I’ve completed the CPC and need to be sent one?

on the cpc card it will get sent off to you by dsa
on the licence you send your one off with pass cert and the new one will come back
the tachograph card you need to send off an pay for one of them you could do this now
