I have dealt with hundreds of these crossings and 99.9% of the time any accident is down to user abuse, not calling the controlling signaller. Its a massive problem on the railway. People either can’t be bothered or ‘think’ they know when a train is due. I know of no instance when it has been signaller error, telling someone they are safe to cross when in fact it is not.
