Annual test.

The lorry is in for its first one tommorow.
Cant believe its a year old already(probably because of the amount of time its spent in the workshop)
Anyway, considering the amount of time they want to prep one nowadays, test failures must be a rare occurrence in this day and age ?
Also, has anybody ever had a lorry fail its first test?

All sorts of things can go wrong and just pack up between the workshop and the testing centre or an air leak suddenly develop. Best chance is always if it’s been on the same brake rollers, used in the test, as a voluntary first. Some makes even with new pads and chambers they can be very close on park brake efficiency and results vary on different rollers although they’re supposed to be calibrated.

I don’t personally think first time pass rate is a good metric of a good maintenance regime all that tends to happen now is, as you suggest, a disproportionate amount of time goes into MOT prep probably to the detriment of other general maintenance work.
