Hi All and @DieselDave,Thanks for getting back in Touch the HSE have not and im chasing my tail every time i contact them, To my knowledge they do not have a record of me calling and stating that my Uncle has passed… i compiled two riddor forms and FAI, from when he passed on Jan 2020 (I done this in Feb) neither the Company nor authorities had notified them, which as you can imagine made me upset.

I think that what got a few of us thinking was your typo ‘2019’ (instead of 2020) in your first post because that would have been unusually long period for no details to be available or answers given to you.
I can’t speak for the HSE, nor do I know who did/didn’t report what or when, but I can say that if yours was the first/only report they received, then the apparent delay might be to do with any number of issues connected to the COVID-19 situation. There’s no harm in asking HSE about progress though.

It has been me the full time been contacting the Health and Safety Exec. I have since recorded a complaint with them. As for what you have said in earlier replies, i will be in contact with those channels as its the first and possibly the singular avenue we have been directed. Thank you all

As I said earlier, DVSA have specially trained officers for HCDG, so a good tip in your initial contact/report with them would be for you to mention that your Great Uncle died and was possibly carrying HCDG in a tanker at the time of his death.
You can raise the important question of the company’s reaction (or lack of it) to the fact that your Great Uncle’s vehicle being immobile for an unusual period on the day of his death should have been known and reacted to by his employer, but hasn’t been satisfactorily explained to you or the family.
You should be prepared that DVSA might not get back to you, but they will investigate and then take whatever action is necessary under the law. This process may take some time.
I still feel that your best source of info about the general circumstances of your Great Uncle’s death, the timings of when something was reported, as well as the reactions of the emergency services would be from the Police. You could ask them for an update.
In any dealings with DVSA, Police or HSE you may need to keep a note of the reference number(s) they give you for the case that you’re asking them to look at. They are different organisations and therefore each have their own ways of operating.
