Just looking at the envelope I have to send off tomorrow for same thing. Its the postcode which is important.
Just read the non D4 form very carefully as there’s a bit for example where it asks of you’re renewing lorry or bus, then under that it has a box for renewing photo. Except according tp the booklet, that second option means “I don’t want to renew my hgv / bus” and overrides the top one.
Also ignore the bit about proving identity which looks like you need a passport, except if you ignore the form and booklet, but look on the back of the form notes then you find you ignore that bit except changing name.
Only a committee could create this form! It is absolutely stupid, but we know how stupid DVLA are now.
