Vocational Licence/DQC updating

Due to renew my medical, vocational driving licence and and update my photograph.

  1. Can a DQC card be enclosed to have the image updated at the same time?
  2. Is there a fee for renewing a vocational licence or updating a photograph on the Licence or DQC card?
    Thanks.

From the little I know - I think the photo on the DQC is updated when a new one is issued and it uses the current photo on the driving licence at the time of the new issue
