The wording on the form for the doctor filling in the medical examination report is as follows:
Before you fill in this report, you must check the applicant’s identity and decide if you are able to complete the Vision assessment on page 2.
If you are unable to do this, you must inform the applicant that they will need to ask an optician or optomotrest to complete the vision assessment.
Suggest using another doctor next time, ask around local drivers where you live, they’ll know who does the job properly and for a reasonable price.
