Medical form returned for not including driving licence

I wasn’t aware you had to put your driving licence in the envelope with the D2 and D4 forms and send it in the post? It doesn’t say anything about sending a copy, should I get send in the physical licence then? Also the D2 form has tons of red circles and parts crossed out (done by the DVLA) but it doesn’t say I have to fill in another form or anything, should I just send the same D2 form back or order a new one and start over?

I got my digital tachograph card through though so at least that one was all fine.

EDIT Oh nevermind, I just found the second piece of paper in the envelope which explains the red ink, doh

Yes you need to send your actual licence away with the form. They’ll send it back with provisional entitlement, nothing on the licence itself but it’ll show online when you do a licence check.
