Help needed on this subject please

we recently had a guy lose is wheels on is trailer but yes the management blamed the driver for not doing his checks which is a ■■■■ take ( get out of jail for the management )
is there a way we can obtain a legal report , and a way of proving the mechanic had not torqued the wheels please can you pm me or advice on this thanks :grimacing:

He lossed his wheels :open_mouth:

had the driver been told verbally that the wheels had been off?

was there a written notice placed in the truck stating that the wheels had been off and that after 50km they would need rechecking?

did the person who did the work on the wheels follow procedure ie, torque wheels then wait 30 mins and re torque again?

did the driver not notice that the threads on the wheel studs being more shiney than others thus making it more obvious that something was not right?

at the end of the day once the driver leaves the yard it is his/hers responsibility to make sure that the vehicle they are driving is legal and roadworthy.
