Last time I called them on this issue they said it was one charge for EACH - one for replacement and one for photo renewal - but I’ll check again…
I rechecked and DVLA stated as you did - JUST THE ONE FEE OF £20
So that seems like the best idea - say the photocard is ‘lost’ 
when applying for the 10 year photograph renewal
