It looks good to me 
In the example you need a total of 45 minutes break for the WTD, as long as you don’t at any-time go over 6 hours working time without having a 15 minute break you can have the breaks any time you like.
I wouldn’t recommend leaving the WTD break to exactly 6 hours working time, or the driving break to exactly 4.5 hours driving time, start the breaks before reaching the latest time allowed and you can be sure of not getting an infringement, this applies especially to driving breaks, but apart from that it’s fine.
Thanks for the reply. So if, let’s say, I moved my breaks half an hour back each so that I had some leeway in case I was stuck in traffic etc, that should still be fine :

Yes that would still be fine, in fact it would be better because you would be less likely to go over your time 
I would do this so it splits the work/drive sessions into 3 parts which are about the same between breaks
