On a fleet policy, do you have to specify the reg of every vehicle that you run, much in the same way you do when you insure your car, or is an insurance policy a blanket one?

Might be polciy specific, best ask your insurance agent.
On ours we specify the vehicles we have at the start of the policy and should add any vehicles we have for more than a week, we also have to put them on the MID if we have them for a week or more.
However we are insured for any vehicle “in our control” in the wording of the policy even if they’re not declared on our policy.
