Please use an up-to-date version of the D4 medical form

DVLA generally revise their D4 medical form every year or so. When you go for a medical to renew your licence or apply for your first group 2 lorry or bus licence, you need to take a D4 form along with you for the doctor to complete.
You can tell which version of the form you`ve got because it has a date in very small writing down at the bottom of the front page. The current version is 4/16. The previous version was 11/15.

It is difficult to get hold of the current version because the DVLA website still has the 11/15 version and they still seem to be sending those out with the reminder letters. It doesn`t matter which of these two you use because they only changed two words in the 4/16 version, and of those two changes, one of them was a mistake!

But please dont bring an older version from 2014 or earlier to the examination. There are some quite big differences in earlier versions which means that the questions dont cover all the DVLA rules, and also, because of the way the questions are written it is more difficult for the doctor to fill in the correct answers to the questions.

if you are applying for your first licence, you will probably already have been in touch with a training school, and most training schools will send out an information pack which contains a D4 form. If you already have a licence and are renewing it, DVLA should send you a reminder a few weeks before it becomes due and that will include a D4 form.

If you can`t get hold of a D4 form you can download one from the DVLA website which at the time of writing is the 11/15 version.
